Operational
Analysis is focused on the identification of as much information
as possible about the process and then using this information
to prevent or eliminate all events that cause loss of production
or represent important economic risks.
|
- Operational
Data Collection - Finding out where the problems lie,
prioritizing on the basis of potential or actual cost
- Hazards
and Operability Studies - To develop a preliminary risk
profile for a facility processing hazardous materials
- Human
Reliability Assessments - To optimize handling processes
for hazardous materials
- Performance
and Safety Monitoring - To identify both real problems
and "near misses"
- Root
Cause Analysis and Incident Investigation of Actual and "near
miss events" - A broad spectrum of techniques and
activities which are applied to ensure that the same problem NEVER
occurs more than once. The goal -to eliminate ALL repetitive failures.
- Operational
Assessments - On-site monitoring of operational processes
and use of human factors and human reliability engineering techniques
to enhance operational processes in ways that are non-threatening
to staff. This is particularly important to preventing accidents
and important operational failures.
